2012 ITA Men's Team Indoors - Master Line-Up Submission

Please complete the Master Line-up Form by listing your players in order of ability and submitting by Monday, February 13th, at Noon (Eastern time), the complete list of line-ups will be emailed to participating coaches at approximately 1:00 PM (Eastern) for review.
 
All protests must be sent via email to Troy Venechanos (tvenechanos@itatennis.com) by 9:00 PM (Eastern time) on Monday, February 13th. The National Tournament Committee (NTC) will make final ruling on all protests on Tuesday, February 14th. There will be no appeals accepted for line-ups that are changed by the NTC.

As a reminder, please review the Line-Up Rules for this event (different from the women's event):

1. Master line-ups must be played throughout the event.

2. If a player is removed from the line-up, all players listed below the removed player will move up in position. “Shuffling” of line-up is NOT permitted.

3. In doubles, you may direct line substitute for a sick/injured player as long as the teams are still playing in order of ability. The opposing coach may protest a doubles line-up if he/she feels they are not in order prior to the start of the match.

4. The on-site ITA Games Committee will have final ruling on all on-site protests.

Please write out the first and last name of all players (singles and doubles). You must provide a link to the up-to date records (including Fall results). The direct link to each player's results can be found on the ITA site via the Results page: http://itarankings.itatennis.com/PlayerSchedule.aspx . Once on this page select your Team and then your player. Your player's singles results will displayed by default. Paste the page's address that is displayed at the top of the browser into the corresponding box provided below in the form. 

A maximum of 8 players can be submitted on the master line-up, this limit (8) also applies to the number of players that are permitted to participate in the event. The final 8 players on all line-ups will be distributed at the coaches meeting.
