Entry Instructions (please read carefully):  Send a copy of this entry form and your $50 entry fee (fee includes singles and doubles) to the appropriate tournament director (see master grid for appropriate contact). Note: ITA National Championships (Pittsburgh, Columbus and Indiana) have a $60 entry fee (fee includes singles, doubles and mixed doubles). You will need to send an entry the tournament director at each tournament you plan enter. Make your check payable to the tournament director. Refunds will not be given to players who withdraw less than 10 days prior to a tournament (no exceptions for the refund). Entries will be accepted up until the tournament is full or until match times have been given out, whichever comes first (unless otherwise noted for that particular tournament).

Note: There is NO selection process/criteria for the ITA National Championships hosted by Indiana University August 11-15. Any player who enters on time can participate in the event, given the singles field has not exceeded 96 players (men), 64 players (women) (priority registration given to current college players up to June 1)

I, the undersigned player, hereby apply for entry into the tournaments which I have indicated on this form and agree to abide by and be bound by ITA and USTA rules and regulations.   I hereby for myself, my executor, administrators, heirs and personal representatives, waive all claims of any kind, nature and description, which I may have now or in the future, against the USTA, ITA and ITA Circuit Committee, ITA Tournament Directors, staff, sponsors, and host schools, and sites of each tournament I have entered for any injury sustained by me while traveling to and from or participating in any of the tournaments which I have entered. I understand that I may withdraw from any tournament I have entered up to 10 days prior to the main draw, and after that I shall forfeit my entry fee.  I understand that if I enter doubles without a doubles partner, the Tournament Director will assign me a doubles partner or refund my doubles entry fee if there are no available players.  I understand no entry fees will be refunded if poor weather causes some events not to be completed.

Notes: A match tie-breaker will be played in place of a third set in singles; 8-game pro sets in doubles; collegiate no-let rule used in men’s tournaments. Most tournaments run Saturday to Tuesday, but not all. Check individual listings.
